Social Services Assistant - Whole Health Partner

Your duties can include but are not limited to the following:
Performs a variety of roles that include welcoming Veterans into the local system, orienting them to Whole Health concepts, offering a variety of points of entry into partnership with VHA, coordinating and connecting Veterans to resources and programs based on preferences, assisting Veterans in articulating their goals for health and well-being, learning and practicing new self-care skills, helping them monitor their progress, assisting them in their health plans, supporting them in advocating for themselves to obtain effective services, and providing tools and resources to assist in navigating the Whole Health System. Provides information and advocates for Veterans' personal health goals and plan, and shared goals with clinical care team by partnering with Veterans in completing a Personal Health Inventory and exploring aspects of a Personal Health Plan in group and individual settings. Assists the Veterans to explore core elements of personal health planning, exploration of what matters to the Veteran, self-assessment using the components of health and well-being, identification of goals and potential actions in self-care areas, and identification of support and follow-up to implement a plan. Performs other duties as assigned. Work Schedule:

Experience:
MUST possess one (1) year of specialized experience equipped with the knowledge, skills and abilities to perform successfully the duties of the Social Services Assistant - Whole Health Partner position. To be creditable this experience must be equivalent to the next lower grade level (GS 5) in federal service. Qualifying specialized experience includes serving as a group leader in community or residential recreation or training programs; serving as a counselor in a youth summer camp; serving as an instructor in a training program for the development of job skills; performing work that demonstrated the ability to be responsible for and to lead and train members of a job crew; serving as a group activities leader in the Armed Services; serving as a unit leader-monitor-proctor for a dormitory group in a boarding school or other residential facility; serving as an aid or assistant to professional or specialist staff in such fields as personnel (private or government), employment guidance in a vocational or academic institution, social service, or manpower development; serving as an aid or assistant to professional or associate staff engaged in work of a social services nature in private, public welfare, government program, or community organizations, or in voluntary activities; serving as an aid or assistant to professional or specialist staff working with individuals who are addicted to or abuse alcohol and/or drugs. (This experience must be documented in resume) OR Education:
MUST have successfully completed six (6) months of graduate level education (9 semester hours above a bachelor's degree) or an internship in course matter that must include courses directly related to the work of the Social Services Assistant position. This education must have been obtained in an accredited college or university. (Transcripts Required) OR Combination of Education and
Experience:
MUST have equivalent combinations of successfully completed graduated education and specialized experience as described above may be used to meet total qualification requirements for this grade level. The education portion must include graduate courses directly related to the work of the Social Services Assistant position. A full year of work is considered to be 35-40 hours of work per week. All experience listed on your resume must include the month and year start/end dates. The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
